SAFETY CONTRACT

To
Whom It May Concern:
I, [MakersDozn], hereby agree not to take my own life, not to cause myself
life-threatening injury, and not to put my life and/or safety at risk.
The
aforementioned agreement shall be binding upon all self-states, known and
unknown, and shall remain in full force and effect in perpetuity.
This
agreement shall not preclude me from establishing a Do Not Resuscitate (DNR)
agreement, a Health-Care Proxy agreement, and/or other health-related agreements
for medical purposes.
I
hereby agree to be fully truthful in all self-states, known and unknown, in
entering into this agreement.

We will be giving the original signed document to our T and will keep a PDF file in our possession.
